When would you like to take appointments (if taking appointments). Please add the time slots one line at a time, such as:

9:00 - 12:00 p.m. Monday - Friday

1:00 p.m. - 4:00 p.m. Monday - Friday

9:00 - 12:00 p.m. Saturday

Do people need to show up early for an appointment? If so, how early? If yes, please indicate as follows:

"Yes. 10 Minutes Early" Early slots can be anything you want, 5, 10, 15, 30 minutes, etc.

This is the maximum number of simultaneous appointments you can handle at one time. If you have multiple salespeople please take this into consideration so we can schedule accordingly.

Are you in fitness? Do clients need to bring, boxing gloves, a yoga mat, towel, running shoes, etc?
